Instructions for joining AFEU

1. Download and print the form.
2. Fill out the following fields:
   1. Name of Employee
   2. Employee Identification Number (e.g. AR012345, not SSN)
   4. Home Address
3. Sign and date the form at the bottom (Signature of Employee)
4. Send the completed form via interoffice mail to the mailstop "AFEU" or hand-deliver to an officer of the local at a regularly-scheduled meeting (every Wednesday at noon.)
